Abstract This is a case report of a large paravalvular abscess of the aortic valve (AV) causing severe aortic insufficiency, complicated by aortic root to right ventricular fistula, in a young patient who admits to intravenous drug use (IVDU) and presented with skin abscesses and septic shock. Our case highlights the cruciality of early detection, diagnosis, and management of aorto-cavitary fistulas by echocardiography, due to high mobility and mortality associated.
