Targeted Use of Postoperative Discharge Phone Calls to Reduce early Readmission Rates

One fifth of PODPCs identified a concern, and over 90% were managed by an NP over the phone or in clinic.•Take Home Message: Postoperative discharge phone calls are feasible and may reduce readmissions among vascular surgery patients.
