•Electronic Health Record (EHR) systems are increasingly being adopted.•Fifteen percent of the reviewed EHR charts contains documentation errors related to cancer diagnosis and treatments.•Patient engagement tools and single source of cancer diagnosis within modern EHR systems should be implemented to improve documentation accuracy.
