The field of heart transplantation has always faced the challenge of donor heart scarcity. For decades, static cold storage (SCS) has served as the standard method for donor heart preservation, albeit with numerous drawbacks. The TransMedics Organ Care System (OCS) Heart emerges as a novel alternative, employing normothermic perfusion to sustain the donor heart in a near-physiological state prior to transplant. This manuscript aims to present a compelling argument favoring the OCS over traditional SCS. Additionally, it explores the OCS's potential to expand the donor pool, enhance outcomes, and change the paradigm in donor heart preservation.
