Spécificités de la prise en charge péri-opératoire du patient en situation d’obésité en chirurgie digestive : recommandations pour la pratique clinique de la Société française de chirurgie digestive (SFCD)

La Société française de chirurgie digestive a souhaité proposer des recommandations pour la pratique clinique portant sur les spécificités de la prise en charge en chirurgie digestive du patient adulte en situation d’obésité. L’analyse de la littérature a été effectuée selon la méthodologie GRADE® (Grading of Recommendations Assessment, Development and Evaluation) en identifiant 5 champs suivants : prise en charge préopératoire ; modalités de transfert et d’installation au bloc opératoire ; spécificités en chirurgie laparoscopique ; spécificités pour la chirurgie ouverte ; prise en charge postopératoire. Chaque question a été formulée selon le format PICO (patients, intervention, comparison, outcome). La synthèse des experts et l’application de la méthode GRADE ont abouti à 30 recommandations. Parmi ces recommandations, 3 ont un accord fort et 9 un accord faible. Pour 18 questions, la méthode GRADE n’a pas pu être appliquée, la recommandation est donc basée sur l’avis d’experts. Ces recommandations pour la pratique clinique pourront aider le chirurgien dans l’optimisation de la prise en charge en chirurgie digestive du patient adulte en situation d’obésité. The French Society of Digestive Surgery (Société Française de Chirurgie Digestive [SFCD]) has elaborated clinical practice guidelines for the management of the obese patient undergoing gastro-intestinal surgery. The literature was analyzed according to the GRADE® (Grading of Recommendations Assessment, Development and Evaluation) methodology divided into five chapters: pre-operative management, modalities of transportation and installation of the patient in the operating room, specific characteristics related to laparoscopic surgery, specific characteristics related to traditional surgery, and post-operative management. Each question was formulated according to the PICO format (Patients, Intervention, Comparison, Outcome). Synthesis of expert opinions and the application of the GRADE methodology produced 30 recommendations among which three were strong and nine were weak. The GRADE methodology could not be applied for 18 questions, for which only expert opinion was obtained. These clinical practice guidelines can help surgeons optimize the perioperative management of the obese patient undergoing gastro-intestinal surgery.
