Abstract
Background:Septal extension grafts are commonly applied techniques in rhinoplasty to correct various nasal deformities. These grafts are generally used to increase the length of the nasal septum, to adjust nasal tip rotation and projection, and to straightening caudal septal/columellar deflections. Objective: The aim of this study is to demonstrate the centralized positioning of the septal extension graft that was suture-fixed in the middle to the caudal part of the septum and its stationing in line with the septum, without moving to the right or left of the septum during rhinoplasty. Materials and Methods: This surgical technique was applied in six cases. In order to increase tip projection and adjust rotation during rhinoplasty, the septal extension graft was sutured continuously to the caudal of the septum with a permanent 5/0 polypropylene suture. The graft was fixed in line with the septum without deflecting to the right or left of the septum. They were followed in average 12 months (between 6 and 18 months), and the results were evaluated. Result: Desired esthetic results were achieved in all patients. Desirable nasal tip projection and rotation were achieved and maintained in these cases. No complications developed in the early and mid-term follow-ups. Conclusion: In our study, we defined a new septal extension graft by suturing the septal extension graft continuously with a permanent polypropylene suture to the caudal septum in line with the septum without fixing it to the right or left.
