•There exist metropolitan and nonmetropolitan disparities in access to advanced temporary mechanical circulatory support therapies.•We identified racial and socioeconomic inequities in access to mechanical circulatory support.•These findings highlight systemic inequities in the care of patients with cardiogenic shock.
