Optimizing the question: Balancing significance and feasibility. Key questions addressed in this chapter include “Will the findings affect change?” and “Can that change be implemented in clinical practice?” Clinical trials involve altering the care of patients and are resource intensive, and thus trials need to have the potential to impact clinical care. A priori optimization of the research question and trial design to balance significance and feasibility is necessary for study completion and translation of findings into clinical practice. Real-world case study examples are presented on the following topics: (1) Designing trials that address a research question of significance and importance, (2) Addressing barriers to accrual and trial completion, and (3) Designing trials conducive to translation into clinical practice.
