Abstract
Implementation of a multidisciplinary protocol for MRI patients with cardiac implantable electronic devices (CIED’s) is vital to improving outcomes, patient experience and increasing patient access. With an average of 1.5 million devices implanted globally each year, a multidisciplinary protocol may be essential to the success of MRI programs. The Allied health professionals are vital to the process and are a key part of a successful program. The ideal protocol involves Radiology physicians, EP physicians, EP Nurse Practitioners, Cardiac Device RN/technologist, MRI technologists, and MRI RN’s. The purpose of this study was to evaluate the implementation of a multidisciplinary MRI protocol in patients with CIED’s. We performed a retrospective analysis of patients with a CIED who had an MRI at our institution before and after implementation of a multidisciplinary MRI protocol. A total of 1058 patients undergoing MRI scans with both conditional and non-conditional CIED's from January 1, 2020-December 1, 2022 were evaluated. Prior to implementing the protocol at our institution, the MRI department performed 129 MRI scans in 2020 in patients with CIED’s. Following implementation of our multidisciplinary protocol in 11/2020 our institution has performed a total of 964 MRI exams in patients with CIED’s. Of these 964 MRI exams, 192 were in patients with non-conditional CIED’s. A total of 359 MRI’s (80 non-conditional) were performed in 2021, an increase of 35% from 2020. A total of 570 exams (112 non-conditional) have been completed in 2022 as of 12/1/2022, an increase of 62% from 2020. No adverse events have occurred. Having a multidisciplinary protocol for patients with conditional and non-conditional CIED’s requiring MRI studies is vital to a successful MRI program. A multidisciplinary MRI protocol allows for a significant increase in volume and therefore patient access with minimal risk of adverse events to patients.
