Countries will need to develop integrated care systems for older people centered on Primary Health Care (PHC) anchored in communities and close to people’s daily lives, characterized by first contact, accessibility, longitudinality, and comprehensiveness. Strong PHC improves people’s health at all ages through disease prevention and health promotion, protects patients and families financially, builds public confidence in health systems, and lowers system costs. But to meet the needs of aging populations, PHC must evolve. The COVID-19 crisis underscored that most countries’ PHC systems remain unprepared for the complex challenges posed by growing numbers of vulnerable older adults, many with chronic comorbidities. Financing, innovation, regulation, and evaluation—the four pillars of the FIRE framework—remain priority levers that all countries can use to strengthen PHC-centered, integrated care for aging populations.
