L’ectropion congénital de l’uvée (ECU) est une anomalie rare du développement embryonnaire du segment antérieur de l’œil. Nous rapportons le cas d’une enfant de 5 ans porteuse d’un ECU non connu et prise en charge en urgence pour une crise aiguë de fermeture de l’angle irido-cornéen (CAFA).Une enfant de 5 ans adressée en urgence pour exploration d’une anisocorie avec mydriase de l’œil droit et céphalées majeures. Une imagerie cérébrale avec injection de produit de contraste est réalisée initialement aux urgences pédiatriques et éliminait une pathologie neurologique centrale. L’examen initial de l’œil droit retrouve une pression intra-oculaire (PIO) à 37 mmHg, un œdème de cornée, un ectropion congénital de l’uvée, une mydriase avec bloc pupillaire, un angle fermé en gonioscopie, un cristallin clair. L’examen de l’œil gauche est sans particularité, notamment sans ECU visible. La prise en charge initiale consiste en un traitement médical par hypotonisants et myotiques locaux et acetazolamide per os à 10 mg/kg/j. La réévaluation sous anesthésie générale montre une mydriase persistante et l’absence de levée du bloc pupillaire. Une chirurgie filtrante est réalisée devant l’absence de réponse complète au traitement médical, permettant un contrôle de la PIO sans traitement hypotonisant et la régression complète de l’œdème cornéen.L’ECU est une malformation rare, et les complications pressionnelles représentent une proportion minime dans les cohortes de glaucomes pédiatriques. La présentation aiguë du CAFA dans ce contexte, potentiellement cécitante à court terme, rend cependant difficile le dépistage et la prise en charge chez des enfants souvent jeunes et très algiques. Seulement un cas similaire a été rapporté dans la littérature chez l’enfant.Une CAFA compliquant un ECU est exceptionnelle, et de diagnostic difficile dans un contexte pédiatrique. Les parents d’enfants présentant cette atteinte prédisposante doivent être informés de la nécessité de consulter en urgence devant l’apparition de signes cliniques d’hypertonie oculaire.Congenital ectropion uveae (CEU) is a rare anomaly of the embryonic development of the anterior segment of the eye. We report the case of a 5-year-old child with an undiagnosed CEU who was treated urgently for an acute angle closure attack.A 5-year-old child was referred urgently for evaluation of anisocoria with mydriasis of the right eye and severe headache. Brain imaging with contrast injection was initially performed in the pediatric emergency department and ruled out central nervous system pathology. The initial examination of the right eye revealed an intraocular pressure (IOP) of 37 mmHg, corneal edema, congenital ectropion uveae, mydriasis with pupillary block, a closed angle on gonioscopy, and a clear lens. The examination of the left eye was unremarkable, with no visible CEU. The initial management consisted of medical treatment with topical glaucoma drops and miotics and acetazolamide at 10 mg/kg/d. Re-evaluation under general anesthesia showed persistent mydriasis and no resolution of the pupillary block. Filtering surgery was performed in the absence of a complete response to medical treatment, allowing control of IOP without drops and complete regression of the corneal edema.CEU is a rare malformation, and pressure complications represent an insignificant proportion of pediatric glaucoma cases. The acute presentation of acute angle closure in this potentially blinding short-term setting, however, makes detection and management difficult in very young children in a great deal of pain. Only one similar case has been reported in the pediatric literature.Acute angle closure complicating CEU is exceptional and difficult to diagnose in a pediatric context. Parents of children with this predisposing condition should be informed of the need to consult urgently when clinical signs of elevated intraocular pressure appear.
