Comparison of COVID-19 Vaccination Rates Among Psoriasis Patients Globally Using the Global Healthcare Study on Psoriasis (GHSP)

Abstract
National guidelines in most countries recommend COVID-19 vaccination for all psoriasis patients, including those on biologics.1 This study compares the rate of COVID-19 vaccination by country and by treatment regimen. We conducted a cross-sectional study (January 2020-July 2022) using the Global Healthcare Study on Psoriasis survey. Psoriasis patients who did not have access to vaccines at the time of survey were excluded. A total of 282 psoriasis patients across the United States (US) (80), Chile (31), China (77), Switzerland (39), and Singapore (55) were surveyed regarding COVID-19 vaccination status. 175 patients (62.1%) were fully vaccinated for COVID-19 (Chile: 93.6%, China: 18.9%, Singapore: 100%, Switzerland: 21.1%, US: 86.3%). Multivariate analyses were adjusted for gender, race, education level, insurance type, and previous COVID-19 diagnosis. Compared to other countries, the US was 4.8 times more likely to be fully vaccinated against COVID-19 (OR: 4.8; 95% CI: 1.6-14.5); China was 0.03 times less likely to be fully vaccinated (OR:0.03; 95% CI: .01-.1); Switzerland was 0.06 times less likely to be fully vaccinated (OR:0.06; 95% CI: .01-.3). Compared to patients on biologics, patients on phototherapy or oral systemics had similar vaccination rates (p=0.33, 0.63). Patients on topical therapies alone were 7.2 (95% CI: 2.0-25.9) times more likely to be fully vaccinated than those on biologics. Country of origin and treatment regimen are associated with differential vaccination rates against COVID-19 in psoriasis patients. Clinicians should be aware of this vaccination gap in psoriasis patients on systemic medications globally and recommend COVID-19 vaccination for all psoriasis patients.
