A cutaneous dermatofibroma is a benign fibrohistiocytic tumor that may present with varied dermoscopic features based on the histologic subtype. Dermatofibromas appear different depending on the light source (polarized vs. nonpolarized) and dermoscopic technique (contact vs. noncontact) employed. Multiple dermoscopic features and patterns have been reported in dermatofibromas. The classically described presentation using nonpolarized dermoscopy is that of a central structureless white patch (previously termed scarlike area) surrounded by a delicate pigment network. In conclusion, a cutaneous dermatofibroma is a benign fibrohistiocytic tumor that may present with varied dermoscopic features. The most prevalent pattern is that of a delicate peripheral network surrounding a central white structureless patch that variably displays vessels and shiny white structures with polarized light. Skin type/color, anatomic location, and secondary epidermal changes may affect the pattern visualized on dermoscopy.
