Calcifilaxis: actualización y serie de casos

espanolLa calcifilaxis es una compleja enfermedad que se desencadena debido al desequilibrio e interaccion entre diferentes condiciones y variables, la mayoria de ellas presentes en pacientes con enfermedad renal avanzada, aunque no exclusivamente. Y se caracteriza por la aparicion de lesiones subcutaneas que evolucionaran hacia ulceras necroticas muy dolorosas. Se presentan datos de 4 pacientes con calcifilaxis, todas mujeres, con una edad media de 67,2 anos con enfermedad renal en tratamiento con hemodialisis o dialisis peritoneal. Las lesiones de todas ellas cursaron con ulcera necrotica, asi como tambien se pudieron apreciar areas de livedo y lesiones nodulares. Se confirmo el diagnostico mediante biopsia en todos los casos. Se aplico cura en ambiente humedo y desbridamiento cortante en todas las pacientes. En uno de los casos se incluyo desbridamiento quirurgico y terapia de presion negativa. La evolucion fue mala, se produjo el fallecimiento de todas las pacientes en un tiempo medio desde el diagnostico de 1,3 meses. A pesar de las incertidumbres que plantea la calcifilaxis en cuanto a su patogenia, diagnostico, tratamiento y evolucion, parece necesario establecer pilares centrales en el tratamiento de las heridas que inevitablemente cada paciente sufrira, fijando como prioridades fundamentales la prevencion de la infeccion, el manejo del dolor y el confort. EnglishCalciphylaxis is a complex disease that is triggered due to the imbalance and interaction between different conditions and variables, most of them present in patients with advanced kidney disease, although not exclusively. And it is characterized by the appearance of subcutaneous lesions that will evolve towards very painful necrotic ulcers. We present data from 4 patients with calciphylaxis, all women, with a mean age of 67.2 years with renal disease undergoing hemodialysis or peritoneal dialysis. The lesions of all of them occurred with necrotic ulcer, as well as areas of livers and nodular lesions. The diagnosis was confirmed by biopsy in all cases. Cure in wet environment and cutting debridement in all patients. In one case, surgical debridement and negative pressure therapy were included. The evolution was poor, the death of all patients occurred in an average time since the diagnosis of 1.3 months. Despite the uncertainties of calciphylaxis in terms of its pathogenesis, diagnosis, treatment and evolution, it seems necessary to establish central pillars in the treatment of the wounds that each patient will inevitably suffer, setting as a priority the prevention of infection, management pain and comfort.
