Empirical study of chronic diseases administration in communities

Objective To explore the management methods and rules for chronic diseases. Methods 80 patients with diabetes and hypertension were selected who came from the East and Wellness Flower Districts of Development Zone of Zhongshan City in Guangdong Province. 4 stages such as quantitative exercise, quantified diet, quit alcohol limit and psychological counseling were intervened and managed. The HJK-H05 chronic disease management system was employed as implementation tools, body mass index(BMI), waist circumference, 2-hour postprandial blood sugar and qualified rates were observed as indicators. Results The BMI, waist circumference, 2-hour postprandial blood glucose and other index of patients after management significantly improved compared with before management, the differences were statistically significant(P 0.05). And the qualified rates of diabetic patients and hypertension patients before management(42.5%, 55.0%) increased after management(75.0%, 87.5%). Conclusion Implementation of HJK-H05 chronic disease management system has better effects for the chronic diseases patients.
