•We validated PREDICT in young, systemic therapy-naïve women with N0 breast cancers.•PREDICT significantly underestimated mortality in these women.•Model discrimination was moderate for women with ER-positive tumors.•Model discrimination was poor for women with ER-negative tumors.•The clinical utility of PREDICT in young women with N0 breast cancers was limited.
