Prognostic prediction index and comparison for ipf

SESSION TITLE: Diffuse Lung Disease Posters 3 SESSION TYPE: Original Investigation Posters PRESENTED ON: 10/10/2023 12:00 pm - 12:45 pm PURPOSE: Idiopathic pulmonary fibrosis (IPF) is a progressive and irreversible disease that shows a poor prognosis and high mortality rates. There are several prediction prognosis indexes, but none of them is gold standard until now. We made a new prognostic index system with several physiologic data and compared its outcome to other prediction tool. We tried to construct new prognostic index to predict the prognosis for IPF. METHODS: 137 IPF subjects were enrolled. We constituted the prognostic index (BDDE) with four clinical profiles: the body mass index (B), the diffusion capacity (D) and dyspnea (D), and exercise capacity (E), measured by the six-minute–walk test. We classified subjects into three categories (high, mid and low) according to BDDE score, and performed the Cox-regression analysis. We compared BDDE and the other prognostic scoring system (GAP score) in according to each quartile group. RESULTS: There were significant association between BDDE score and total walking distance in 6-MWT.(R2=0.52, P<0.01) Subjects who showed hypoxemia during 6-MWT had higher BDDE scores than non-hypoxic patients.(P<0.01) Higher BDDE score group showed higher risk for death than other groups. (HR 4.59, P<0.001) There was a significant difference among group that concordant of quartiles of BDDE and GAP and groups discordant in survival analysis. (P=0.003) While there was no significant difference in the between discordant groups. More predictable for outcome when the quartile of BDDE score and GAP was concordant. CONCLUSIONS: Our new modified prognostic grading system, BDDE index, would provide better and more precise prognosis in IPF. More predictable outcome could be obtained when multiple prognostic index used. CLINICAL IMPLICATIONS: This simple model can be used to predict the prognosis of IPF patients.
