Orthodontic-orthognathic treatment combined with premaxillary distraction osteogenesis of an adult patient with isolated cleft palate

Maxillary hypoplasia is a craniofacial characteristic in patients with isolated cleft palate (ICP). For maxillary hypoplasia with dental arch length deformity and severe crowding, premaxillary distraction osteogenesis can correct maxillary hypoplasia and increase the dental arch length to eliminate crowding. This case report describes the interdisciplinary treatment of a 20-year-old female with ICP with severe maxillary hypoplasia, mandibular prognathism and asymmetry, severe maxillary crowding, missing teeth, midline discrepancy, and anterior crossbite. An interdisciplinary treatment plan that included comprehensive orthodontic treatment, premaxillary distraction osteogenesis, mandibular orthognathic surgery, implant surgery, and endodontic and periodontal treatment was implemented to correct craniofacial deformity and malocclusion. A harmonious facial profile with satisfactory occlusion was obtained at the end of the interdisciplinary treatment. At 1-year follow-up, the treatment outcomes were stable. This case report aimed to demonstrate that interdisciplinary collaboration and excellent patient compliance are essential for patients with ICP to achieve functional occlusion and harmonious facial esthetics.
