•Stroke, death, and MI were similar between open TR and ER SAT reconstruction.•TR patients had more sepsis, transfusions, non-home discharges, and longer LOS.•Patient selection for TR appears reasonable in this contemporary analysis.•These outcomes should inform hybrid and endovascular considerations.
