Abstract
There are many options for solving complex upper extremity soft tissue defects in the setting of underlying fractures. Understanding the patient's injuries and exposed structures is critical for selecting the appropriate reconstruction. DRT has allowed for a bridging of many wounds that are not amenable to skin graft coverage. If a flap is required for coverage, the defect size and location are the greatest factors for the appropriate operative treatment. Often times, multiple options are available for reconstruction and the decision must be made in conjunction with the patient for each specific clinical encounter.
