Population pharmacokinetics and model-based dosing optimization of teicoplanin in elderly critically ill patients with pneumonia

•Suboptimal teicoplanin exposures are common in elderly critically ill patients.•Teicoplanin (TEC) PK are substantially altered in elderly critically ill patients.•Impaired renal function results in slower clearance and larger volume of TEC.•Adequate exposures are not achieved with standard dosages of TEC for pneumonia.•High-dose TEC regimens should be considered as empiric therapy or at higher MICs.
