•Hemiarthroplasty dislocation can present as a serious complication associated with high mortality in the elderly.•Even though various treatment options are available, there is no effective management strategy.•Conversion of dislocated hemiarthroplasty to a constrained total hip replacement gives good functional and improved morality rates.•It prevents recurrent dislocation and avoid all the complications of prolonged bed rest.
