•Radical resection of isolated lung metastases from colorectal cancer is still debated.•Five clinical features act independently in the prognostic outcome of CRC patients.•The Meta-Lung Score is an outcomes predictor-tool in CRC lung metastasectomy.•Upfront surgery could be considered in patients scoring 0 and 1.•Prospective randomized clinical trials are needed in patients with a score from 2 to 5.
