Abstract
Degenerative mitral regurgitation is the second most frequent indication in cardiovascular surgery. Barlow's disease continues to be a challenge for surgical repair. The complexity and deformity of the anatomical valve structure, anatomopathological changes that involve the valve tissue and the fibromuscular skeleton, with alterations in the ventricular architecture make its repair difficult. It requires precise knowledge of the disease and its anatomical, functional, and haemodynamic aspects to allow us to see beyond the clinical presentation of the patient to offer the best treatment.
