Do-It-Yourself Automated Insulin Delivery: A Position Statement

Type 1 diabetes (T1D) is a complex chronic condition characterized by a complete lack of endogenous insulin production. Despite advances in insulin analogues and glucose monitoring systems, living with T1D remains burdensome [ [1] Weisman A. Automated insulin delivery (AID) systems have been proven in randomized trials to help reach glycemic targets while reducing the burden of self-management for people with T1D and their caregivers (PWD ∗ Throughout this statement, the term PWD will be used to refer to people living with T1D and their caregivers. ). AID systems rely on a computer-based algorithm to modulate insulin delivery by an insulin pump based on glucose levels derived from real-time continuous glucose monitoring (rtCGM) data. This real-time adjustment of insulin delivery has been shown to ease burden and increase the safety of living with T1D, and so the American Diabetes Association (ADA) has recently added a standard of care statement that commercially approved AID should be offered to youth and adults with TID who can use the devices safely [ [2] American Diabetes Association Professional Practice Committee7.
