•It is the first meta-analysis of OCT diagnoses breast cancer margins during breast-conserving surgery.•OCT showed high sensitivity, specificity and AUC.•OCT is a real- time, label-free and non-destructive imaging technique.•OCT has the potential for intraoperative margin assessment in breast cancer patients.
