Successful Treatment of Acute Antibody-Mediated Rejection of Liver Allograft with Imlifidase

•The incidence of acute antibody mediated rejection in liver transplantation is low, however the consequences of acute antibody mediated rejection often leading to severe fibrotic changes and early graft loss.•Management of refractory, severe antibody mediated rejection remains without a clear gold-standard approach.•Imlifidase, an IgG degrading enzyme, has been successfully used in highly desensitized kidney transplant recipients.•This is the first case report documenting treatment of refractory AMR in liver transplant patient utilizing imlifidase.
