•The incidence of symptomatic radionecrosis is higher when combining T-DM1 and brain RT compared to radiotherapy alone.•The combination of T-DM1 and locoregional RT for non-metastatic disease is associated with a low rate of side effects.•The combination of T-DM1 and RT shows safety in terms of acute skin effects and pneumonitis.•The field is currently awaiting international consensus recommendations.
