•Octogenarians after SSRF had low inpatient mortality of 6%.•90% of patients were narcotic free at 90 days.•90-day mortality was double inpatient when including chest wall injury related deaths.•An elevated rib fracture frailty index (RFFI) appeared to be associated with overall 90-day mortality suggesting this index as a more useful tool for preoperative risk stratification in comparison to other patient characteristics.
