•BMI was nonlinearly related to all-cause mortality, with the highest risk being presented by the underweight group.•The obesity paradox existed within the non-type 2 diabetes subgroup.•BMI had a nonlinear negative relationship with mortality in non-type 2 diabetes populations.•In type 2 diabetes, there was a U-shaped relationship between BMI and mortality.
