•Well-circumscribed masses are very common in breast cancer screening.•Most well-circumscribed masses have a low positive predictive value for malignancy.•Identifying solitary, new or growing lesions is key to recognizing malignancies.•Comparison to prior clinical examinations may improve positive predictive value.•Recall at subsequent rather than initial screening, if grown, could be considered.
