基于医联体的我国老年医疗服务链断层修复路径研究

Abstract
我国老年医疗服务链中院中急性期照护和长期照护之间出现了断层状态,中期照护的缺失不仅造成了老年患者非计划性重返病房,还会留下不同程度的功能障碍.本文为修复断层的老年医疗服务链提供路径参考,提出在医联体内基层医院中构建中期照护模式,通过"互联网+"衔接并整合医疗资源,更好的为老年患者提供完整性的服务,打造全生命周期的老年医疗服务链.
