No tolerance for food intolerance

70 male with a history of celiac disease admitted for cardiac catheterization. Post-catheterization he received a meal with gluten resulting in significant vomiting and diarrhea. Despite history of celiac, the patient’s gluten intolerance was not documented in the system leading to the dietary error. Omission of food intolerance documentation in the EMR is likely common but rates are not well described
