Diabetes-related outcomes with basal-bolus vs. premixed insulin among veterans with type 2 diabetes: A single institutional retrospective study

Background : Thepurposeofthisstudywastocomparediabetes(DM)-relatedoutcomesbetweenbasal-bolus(BB) andpremixed( PM) insulinregimens. Methods: Retrospectivechartreviewincludingveteranswithtype2diabetes(T2DM),>= 18yearsoldwithhemoglobinA1c( HbA1c)>= 8%. OutcomeswereassessedafteroneyearofBBorPMinsulintherapy. DatawereanalyzedusingChi-square/ Fisherexacttestsandlogisticregression. Results: Outof140enrolledsubjects(70BBand70PM), 94% weremaleswithaverageageanddurationofDMof65.7 +/- 10.1and12.9 +/- 9years, respectively. TheBBandPMgroupsweresimilarinage, genderdistribution, HbA1c, bodymassindex( BMI) andDMdurationatbaseline. Following1yearoftreatment, therewasnosignificantdifferencebetweenthegroupsforchangeinHbA1c(-1.9 +/- 1.8vs-2.1 +/- 1.9%, p= 0.3) orhypoglycemiarate(30% vs21.4%, p= 0.3), respectively. TherewassimilarincreaseinaverageBMIinbothgroups(0.84 +/- 3.1forBBvs0.4 +/- 2.2kg/m2forPM, p= 0.2). Conclusions: Therewerenosignificantdifferencesforglycemiccontrol, hypoglycemiarateorBMIbetweentheBBorPMinsulingroups. TheseresultssuggestPMinsulinisequallyeffectiveandsafeasBBinsulin.
