Self-management is key to the management of many different chronic conditions. For patients with chronic kidney disease (CKD) and cardiovascular disease, the aim of treatment is to enhance quality of life through self-management. It is vital that patients and their care partners/family members are provided with the necessary knowledge and skills to make informed decisions about their health, wellbeing and life goals. For patients with both CKD and cardiovascular disease, living well requires good family support and a dedicated interdisciplinary team. Nurses have a key role to play here. The conceptual framework of living well with kidney disease and its principles can be applied to patients with both CKD and cardiovascular disease. This framework relates to various self-care themes, including participation in life, patient empowerment and engagement, patient activation, patient-centred care and a strengths-based approach. These themes will be discussed, along with suggested strategies, highlighting that patients should be able to live well with their chronic conditions in conjunction with ongoing support.
