Prognostic value of total thiol and D-dimer in patients hospitalized with COVID-19

Objectives: The lack of specific treatment for COVID-19 and the fact that the clinical course differs between individuals makes it difficult to predict the prognosis. The aim was to investigate the prognostic value of total thiol, D-dimer, procalcitonin (PCT), ischemia-modified albumin (IMA), and complete blood count (CBC) in patients hospitalized with COVID-19. Methods: 100 consecutive patients were hospitalized with COVID-19, confirmed by RT-PCR between December 2021-March 2022 and 30 healthy control participated in the study. According to the World Health Organization guideline, two groups were created as critical and non-critical. D-dimer, PCT, IMA, total thiol levels, and CBCwere analyzed. Receiver-operating characteristic curves (ROC) were utilized to determine an optimum cut-off value for distinction. Results: We defined a cut-off value of 1,030 mu g/L for D-dimer (Area Under Curve, AUC): 0.691; p=0.001) and 148 mu mol/L for total thiol (AUC: 0.749; p<0.001) via ROC analysis. The combination of D-dimer and total thiol reached 65% positive predictive value (PPV) and 80% negative predictive value (NPV). Conclusions: D-dimer and total thiol may help predict critical patients with COVID-19.
