Question: A 62-year-old man underwent colonoscopy for rectal bleeding, iron deficiency anemia with a hemoglobin level of 6.5 g/dL, and hypoproteinemia with total protein level of 51 g/L (reference range, 64–83 g/L), albumin level of 38 g/L (reference range, 40–49 g/L), and a prealbumin level of 0.16 g/L (reference range, 0.2–0.4 g/L). Medical history included left colon diverticulitis with perforation, renal transplantation for p-antineutrophil cytoplasmic antibody–associated necrotizing glomerulonephritis with mycophenolic acid, tacrolimus, and methylprednisolone maintenance treatment, hydrocele, and orchi-epididymitis.
