•In a sample of those using opioids in NJ, 15 % overdosed during COVID-19•More than 30 % relapsed during COVID-19 after a period of sustained abstinence•Relapse and medical insurance loss each tripled the pandemic-related overdose risk•Overdose risk was also tripled by the use of non-prescribed Suboxone.
