ObjectivesThere is growing evidence that ADHD may be associated with hoarding disorder (HD). In a review of the literature comparing individuals with OCD only and OCD+HD, 5 out of 14 studies with adults and 3 out of 6 studies with children endorsed this association. Following this model, we analyzed a large sample of individuals participating in the Genomic Psychiatry Cohort OCD research study (GPC-OCD) between 2015 and 2021 throughout the United States and Spain.MethodsOur sample consisted of 1001 adult individuals with OCD-only and 579 adult individuals with OCD and probable HD (OCD+HD). A range of psychiatric diagnoses was determined by semi-structured interviews and self-report surveys. All individuals were diagnosed with OCD using the Florida Obsessive-Compulsive Inventory (FOCI) and Yale-Brown Obsessive-Compulsive Scale (YBOCS). Informed consent was obtained from all participants, and an IRB approved all procedures. Descriptive statistics and frequencies were calculated.ResultsThe 2 groups had similar sex distribution (62.7% females in the OCD-only group vs 59.2% in the OCD+HD group; p = .345). There was no significant difference in the YBOCS score or in the age of diagnosis, onset, first interference, or first treatment for OCD. Individuals with OCD+HD had poorer insight and were more likely to live alone or in supervised living and to have fewer years of education (p < .001). This demonstrates the impact of OCD+HD on relationships, academic performance, and quality of life. The prevalence of lifetime diagnosis of ADHD was higher in the OCD+HD group (19.9% vs 15.3%; p < .001). Furthermore, there was also a high rate of symptoms of inattention (50.2% vs 40%; p < .001) and hyperactivity-impulsivity (45.8% vs 33.4%; p < .001) in OCD+HD.ConclusionsOur overall sample had a higher frequency of lifetime diagnosis of ADHD, being more frequent among individuals in the OCD+HD group. However, it is important to note that we found an even higher frequency of individuals who had undiagnosed ADHD but reported significant symptoms at the moment of the interview with inattention and/or hyperactivity, which are 2 of the core features of ADHD.
