•IFIs after CAR-T treatment have been poorly studied. This study was conducted to fill this gap by screening risk factors, as well as the impact of IFIs on one-year survival of patients.•Prolonged duration of lymphocyte deficiency, CRS grade, and ventilation are linked to fungal breakthrough in patients with hematologic malignancies 60 days after CAR-T infusion.•Although IFIs and patient survival at 1 year are not correlated, the occurrence of IFIs reminds clinicians to optimize the established prevention plan for medical costs and other considerations.
