Abnormal superior oblique (SO) tendons have been characterized in both congenital and acquired palsies. The nasal insertion variant (class II) is rare but usually associated with a loose tendon in congenital cases. Iatrogenic anteriorization and nasal displacement of the SO can also occur following ocular surgery.
