•The APPLE study is a post stroke cohort study with inclusive recruitment and follow up at 1,6,12 and 18 months.•Data include assessment of pre-stroke cognition, prospective neuropsychological assessment, baseline imaging and a biobank.•The APPLE materials (data, biobank, imaging) can be made available as a resource to other researchers.
