•Ruptured craniopharyngioma presents with a suprasellar mass and drop lesions in the spinal canal.•The characteristics of ruptured craniopharyngiomas can be radiologically indistinguishable from metastatic cancer.•Histological diagnoses are important in directing the management of sellar/suprasellar lesions with atypical features.
