Concordance of experienced-based maintenance warfarin dosing vs. algorithm-based dosing

•Experience-based warfarin dosing matched an algorithm only about half the time.•Algorithm-concordant dosing increased the likelihood of returning the INR to range.•The larger the INR deviation, the lower the likelihood for dosing concordance.•No provider characteristics correlated with increased rates of return to INR goal.•Additional research of warfarin dosing algorithm use at extreme INRs is recommended.
