•The favorable treatment outcome of perforated peptic ulcer disease is 85.8%.•Khat (scientific name Catha edulis) chewing is a risk factor for PPUD.•Delayed presentation is significantly associated with mortality in perforated PUD.•Post-operative complications are common causes of prolonged hospital stay and mortality in perforated PUD.
