P5-6 Clinical feasibility and current trends of approved multigene panels in a genomic medicine cooperative hospital

Three comprehensive multigene panels were approved in Japan and might provide a comprehensive genomic profiling for solid tumors and help us to choose matched treatments for the patients. We summarized the clinical utility and current trends of these panels in a genomic medicine cooperative hospital. We have examined 87 cases who had advanced solid cancers of various origins (FoundationOne® CDx/FoundationOne® Liquid CDx/OncoGuideTM, 69/16/2). Physician‘s opinion based on the obtained gene profiling data was discussed in the expert meeting before it was provided to each patient. Mean age of all patients was 59.4 years old (range; 19-80). Genomic profiling data of 78 patients (89.7%) have been obtained. 9 (10.3%) were withdrawn because of sample insufficiency. All cancer types but head and neck cancer were detected at least one actionable alterations, and 74 patients (94.9%) harbored actionable alterations. 5 patients (6.4%) had high tumor mutation burden, and one patient was detected microsatellite instability. 27 patients (34.6%) consequently suggested specific treatments according to their genomic findings. Among them, the patients of some cancer types, e.g. prostate cancer, gastric cancer, thyroid cancer, had high opportunities to find genomically matched therapies. 7 patients (9.0%) finally reached the expert-recommended treatments including approved drugs, off-label drugs for unapproved indications, and ongoing clinical trials of the gene specific molecular targeted therapies. Other 4 patients died at returning the results. 10 patients (12.7%) were referred for genomic counseling due to incidental secondary findings, however, no one has consulted at present. These results were comparable with the previous reports from a Japanese high-volume institution. These multigene panels might provide opportunities of molecular specific therapies for the patients, however, further data accumulation and careful follow-up is needed.
