IDDF2022-ABS-0121 The recurrence and quality of life in patients with acute pancreatitis after discharge: a 12-month follow-up study

BackgroundTo conduct a longitudinal observational study to explore factors related to recurrence and quality of life (QoL) in patients with acute pancreatitis (AP) after discharge.MethodsThis is a prospective observational study and we consecutively recruited AP patients from Peking Union Medical College Hospital from July 2018 to July 2021. Social demographic and clinical characteristics were collected before discharge. All eligible patients were invited to complete an online questionnaire about their recurrence, EQ-5D-3L, and SF-36, at the time of discharge, as well as at 1, 3, 6, and 12 months afterwards. Logistic regression analysis with backward selection was employed to explore the risk factors for AP recurrence. Linear mixed models were adopted to assess the factors that influenced QoL during the 12-month follow-up.ResultsIn total, 116 patients were invited to this study, and 109 of them completed online questionnaires (response rate=94.0%). The recurrence was reported by 36 patients (33.0%). Patients with older age (aOR=2.73, 95% CI=1.01–7.39, P=0.048), a past history of pancreatitis (aOR=3.75, 95% CI=1.51–9.30, P=0.004), and hyperlipidemia etiology (aOR=3.48, 95% CI=1.13–10.73, P=0.030) had a higher risk of recurrent pancreatitis. For patients without recurrence, the physical component summary (PCS) and mental component summary (MCS) of the SF-36 showed an increasing trend and the scores remained stable after 6 months. However, for the patients with recurrence, the PCS had an increasing trend (39.6, 49.7, 44.7, 53.6, and 52.6), but the MCS remained low (42.4, 40.9, 44.1, 46.8, and 44.6). Linear-mixed models indicated that, in addition to recurrence, a history of diabetes was a factor that reduced the QoL during the 12-month follow-up as well.ConclusionsPatients without recurrence recovered quickly, and their quality of life stabilized approximately six months after discharge. Although patients with recurrence had a rising PCS, they were still worse than patients without recurrence one year after discharge. Besides recurrence, a history of diabetes was also associated with a relatively poor quality of life.
