•The risk for pneumatosis intestinalis (PI) increases in patients with acute gastrointestinal graft-versus-host disease receiving steroids.•The risk for PI increases with supplemental nasogastric tube feeds.•Asymptomatic PI may be self-limiting.•Computed tomography imaging is not necessary for diagnosing PI.•Clinical improvement should outweigh follow-up imaging when resuming diet.
