•Pharyngocutaneous fistula (PCF) is a devastating complication of laryngectomy.•The study investigated the risk and predictors associated with PCF formation.•The PCF rate was doubled, 66.66%, in the re-irradiation group of patients.•Most (75%) fistulas in the re-irradiation group were longstanding or permanent.
