•Hospital intervention (MIACT) linked women who use substances to treatment postpartum.•MIACT increased routine obstetrics visits and receipt of contraception.•Brief intervention postpartum has potential to reduce future substance-exposed pregnancies.•Substance use was higher among women in the MIACT group.•Additional intervention is needed to reduce substance use among postpartum women.
